JOB DESCRIPTION:

Minimum two years recent experience required in position applying for. New Grads & Residents not accepted.

Only accepting providers with a completed EM Residency.

Please submit availability with presentation: Days, Nights or Both, How Many Shifts per Month, how soon can you Start?

MUST be willing to work in our PEDS EM department within the Main ED.

SUBMISSION REQUIREMENTS

• Must provide the earliest date of availability

• Must provide how many days per month are available

• Must provide when a candidate has 100% open schedule

• Must provide any dates provider not able to work in next 4 months

• Must have Texas License and DEA prior to name clear submission

• Must have clean file

AS A REMINDER:

• All candidates must have met the minimum shift availability listed on the job.

• CV must have all dates in MM/YY to MM/YY format

• Work History must be in Sequential order with most recent on top

• Any Gaps over 30 days must be explained on the CV

• EPA (Please use the attached EPA. It’s also in Trio under your Name & then click on Help)

• All Licenses, Boards and Certs with expiration dates must be listed on CV

• Anyone with malpractice must provide a NPDB

• Anyone with legal issues must provide supporting documents

Availability

• 1st possible date they would be available?

• How many shifts would they be able for per month?

• What month is their schedule completely open?

• Any shift preferences (Day, Night, Blocks...etc)?

• Any dates in the next 3-6 months they can’t work?

• Any Holidays in the next 3-6 months they would be open to work.?
