The Palliative Care Physician provides health assessment and direct care in all our practice environments to patients as part of the provider team. The Palliative Care Physician provides comprehensive medical services for patients, regardless of age or sex on a continuing basis by performing the following duties outlined in this document.

EXPERIENCE
Board Certified in Primary Specialty Board Certification or Board Eligible in Hospice and Palliative Medicine with expectation of completion of Board Certification within a reasonable time frame.
LICENSURE
Current Unrestricted CT medical license, DEA, CT Controlled Substance Registration (CSP)
