Summary:

The role of the Medical Scribe / Physician Facilitator in the Emergency Care Center (ECC) includes assisting his or her assigned physician with documentation of the patient encounter, helping with patient flow by watching for results, helping set up for procedures and obtaining items for the physician as needed. He/She is not directly involved in patient care. The Emergency Care Physician Liaison demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission and Values of Covenant HealthCare and the commitment to Extraordinary Care for Every Generation.

Responsibilities:
  • Has social contact with patients, including all age groups.
  • Demonstrates excellent customer service.
  • Familiar with medical terminology and basic anatomy.
  • Follows the physician into patient rooms to document the encounter including the history, review of systems, and physical exam.
  • Updates the patient chart to reflect any changes or corrections to the past medical history, surgical history, or social history as directed by the physician.
  • Gathers information from the patient chart for the physician to review.
  • Displays x-rays or other studies for physician to review. Tees up (pends) any orders (excluding medications) the physician requests, for the physician to sign.
  • Monitors all lab and imaging studies to ensure they are returned timely and notifies the physician when results are ready or if there are delays.
  • Obtains items for the physician as needed and helps set up for procedures, but will not have any direct patient contact.
  • Obtains items for the patient as requested by the physician - like water, food, or a blanket.
  • Completes the ECC visit note, including importing all pertinent studies as directed by the physician.
  • Documents procedures performed, discussions with consultants or family members.
  • Documents any discussions with consultants or family members.
  • Under the direction of the physician, completes the ECC course, medical decision making section, and diagnosis.
  • Ensures completed charting is accurate, legally sound, and captures all pertinent data for reimbursement.
  • Completes discharge instructions, including prescriptions as directed by the physician, for the physician to sign.
  • Maintains active knowledge of key billing and coding practices.
  • Maintains active knowledge of emergency medicine related core measures and monitors for its application.
  • Remains flexible in cooperation with 24 hour staffing patterns and scheduling needs for provision of care for the special needs of the ECC department.
  • Maintains good documentation skills based on regular QA reviews
  • Attends unit conferences, in-service programs, annual mandatory education, competency development and continuing education activities to meet the organization's expectations.
  • Other duties as assigned.

EDUCATION/EXPERIENCE
  • High School diploma required.
  • Currently working toward an advanced degree as a physician, physician assistant, or nurse OR must have completed a scribe, medical terminology, or electronic medical record training program/course OR prior experience as a Medical Scribe.
  • Maintain good documentation skills based on regular QA reviews
  • Attending unit conferences, required competencies, in-service programs, annual mandatory education, competencies and continuing education activities to meet the organization's expectations.

  • Basic computer skills.
  • Knowledge of standard office equipment.
  • Familiar with medical terminology and basic anatomy.

  • Excellent time management, to achieve maximum productivity.

WORKING CONDITIONS/PHYSICAL DEMANDS
  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
  • Constant hearing, near vision and midrange vision.
  • Frequent lifting up to 10 lbs.
  • Frequent standing, walking, sitting, far vision, depth perception, visual accommodation, color vision and field of vision.
  • Occasional lifting, carrying, pushing, pulling, twisting, reaching, handling, feeling and talking.
