Columbia Memorial Hospital (CMH) is located in beautiful Astoria, Oregon, on the rugged north Oregon coast, just 90 minutes from Portland. OHSU is seeking an emergency physician leader to join our team to be staffed in the CMH emergency department (0.50FTE) and provide high technical and service quality patient care oversight as the Medical Director in our Urgent Care clinics (0.50FTE).

The CMH emergency department offers a great opportunity for teaching and practicing high-quality emergency medicine including pediatrics and trauma. Second year emergency medicine residents enjoy a month-long rotation/elective which receives high scores within resident education.

The ED physician group has 7 physicians, 4 physicians are DEM faculty, with 3 living in the community, while 3 physicians are part-time and work at either Adventist Health Portland or Hillsboro Medical Center and CMH. Engagement in the group is exceptional. CMH was recognized as one of the most wired critical access hospitals in the nation. There are two daily physician shifts and the shift length is 12-hours; 6a-6p and 6p-6a. The department is double covered from 12p-10p with an emergency medicine resident or an APP.

CMH Urgent Care Clinics are located in Astoria, Seaside, and Warrenton and care for over 12,000 patients annually with seasonal increases during the summer months. The urgent care clinics manage all age ranges and a standard variety of acute conditions with an emphasis on low risk.

The employed position has a competitive salary range with a comprehensive benefits package. Flexible block work schedule and reasonable clinical volume allow travel/exploration and personal time.

Function/Duties of Position

Core responsibilities including work analysis for position:

Leadership oversight:
  • Provide mentorship to UC providers
  • Oversight of development of UC policies, workflow, and best practices
  • Meet monthly to review clinical algorithms and system processes
  • Annual review of APP onboarding process
  • Create an inclusive, equitable, and diverse work environment

Advanced Practice Provider operational supervision duties include:
  • Oversite of operations related to UC provider activity
  • Manage, implement and optimize the role of advance practitioners within the UC setting
  • Facilitate healthy communication between the Emergency Department, Urgent Cares, and external entities
  • Reviews monthly productivity report, operational metrics, and NRC data (patient/customer satisfaction)
  • Coordinates UC provider incentive process with CFO
  • Annually reviews incentive metrics with CFO and UC leadership

Provider Quality and Safety Review: (aligns with Oregon Medical Board requirements)
  • Perform monthly quality and safety chart audits and case review according to the internal UC QA process as well as in compliance with OMB Supervising Physician requirements for PA Practice
  • Review, document and provide constructive feedback on provider charting and medical management
  • Review UC referrals to ED to identify opportunities for improvement, optimize communication, and highlight strengths
  • Reviews all Verge (patient complaints) associated with UC providers and provides feedback to CMH administration and UC providers
  • Mediates UC provider concerns regarding clinical quality and patient safety within CMH medical staff and operations

Recruitment/retention:
  • Recruit and cultivate medical professionals who will facilitate advancement of department goals by participating with CMH HR and UC leadership team on UC provider recruitment. This includes interviewing and vetting of candidates, and making hiring recommendations.
  • Conduct UC provider annual performance reviews
  • Facilitate UC provider professional development and goal setting with great individual variation based on level of experience and goals
  • Promote core training including FPPE. Assess for potential deficiencies, devise plan for UC provider to gain experience and confidence in their clinical management.
  • Annual review of skills training opportunities for continued learning and engagement
  • Oversight of proctoring plans and individual UC provider progress and compliance
  • Advises CFO and UC leadership on UC provider contracts, including renewal and non-renewal recommendations

APP Clinical Scheduler duties will include:
  • Oversight of the Urgent Care APP clinical staffing structure and schedule to ensure all urgent cares have sufficient staffing and balance between sites is achieved
  • Resolves clinical staffing related issues

Maintenance of credentialing/privileges:
  • The medical director will work with leadership to review and maintain privileges for providers at each clinical site

Program development:
  • Coordinates with UC leadership the continued UC program and services development
  • Participates in CMH UC site development
  • Explores/advises on new UC business opportunities

Required Qualifications

Qualifications or regulatory requirements for the role:
  • OHSU Emergency Medicine physician (Department of Emergency Medicine requirements include BE/BC in EM)
  • Completion of an Administrative Fellowship or previous operational experience, including oversight of Advanced Practice Providers, preferred but not required
  • Oregon State medical license
  • Reside in Astoria, Oregon or neighboring coastal communities
  • Demonstrated ability to satisfy standards of performance delineated by law
  • Ability to manage heavy workload with multiple priorities and inflexible deadlines
  • Ability to communicate effectively verbally and in writing
  • Proficient in the use of computing hardware and software, including personal computing equipment, electronic medical record, word processing and spreadsheet software
  • Regular and consistent attendance at work
  • Current Advanced Cardiac Life Support (ACLS) Certification required; ABEM or ABOEM Board certified or board eligible
  • Ability to complete the credentialing process and be approved by the OHSU Professional Board to practice as an independent practitioner in an acute care/emergency setting
  • Registered with Federal Drug Enforcement Administration and be authorized to prescribe medications prior to start date
